Patient File / Case File Journal

I’ve spent enough time with actual paperwork about my mental health that turning a journal into a patient file just made sense.

There’s something weird about seeing your life reduced to notes, codes, diagnoses, checkboxes, medication lists, and little boxes somebody else fills in about you. Sometimes that information is useful. Sometimes it helps. Sometimes it feels like somebody tried to summarize an entire person on one page and called it good.

So I made my own file.

The cover has the full Patient File / Case File design with the scans, barcodes, redacted sections, research markings, warning labels, and all the little clinical details that make it look like something you probably weren’t supposed to take home.

Except this one is yours.

Use it for therapy notes, brain dumps, mood tracking, symptom tracking, medication notes, random thoughts, things you need to remember, things you definitely should tell your therapist, or just whatever is taking up too much space in your head that day.

Mine would probably be half useful information and half shit I wrote down at 2 a.m. because my brain refused to shut up.

That counts too.

The whole point of The Label Lab is that paperwork can document parts of a person without ever explaining the whole person. This journal fits right into that idea.

Write down what matters.

Cross things out.

Change your mind.

Add another page.
